CASE MANAGEMENT

The Case Management Department conducts an in-depth in-home assessment and works directly with the client and family to develop a care plan tailored to help clients with securing needed services.  The primary goal of the Case Manager is to help clients maintain their independence for as long as it is safely possible.   

        In-Home Services provided by the Shenandoah Area Agency on Aging include:   

        HOMEMAKER – Light housekeeping is provided to homebound clients   
        PERSONAL
CARE – A CNA provides assistance with bathing and other  personal hygiene tasks 

        EMERGENCY FUNDS – This program is designed to provide up to $200.00 payable to the vendor in case client if facing
        a financial crisis   

        HOME REPAIRS – A small home repair budget assists clients with maintaining a safe home environment 

The Case Manager is knowledgeable of alternative supports in the community which may be needed to further assist clients.  Supports may include:  public subsidy, assistance with medication costs, and alternative housing options.
